What is EyeOne Vision Plan?

EyeOne Vision Plan is a Vision Plan Platinum benefit program, available at no cost to you, that has been organized by eyecare Optometrists and Ophthalmologists in your community in order to make affordable eyecare available to you and your family.

 

What is a Vision Plan?

A Vision Plan is benefit offered through insurance companies and independent eye care providers that is developed in order to assist families in having access to affordable eye exams, glasses and contact lenses.

 

What is theEyeOne Platinum Vision Plan?

The EyeOne Platinum Vision Plan is a discount benefit plan that extends to EyeOne members a set of defined benefits that allow you to know the cost of your eye care.

 

What is the cost of membership for me and my family?

EyeOne Platinum Vision Plan is offered to you at no cost through the efforts of local Optometrists and Ophthalmologists.

 

What are the benefits available to me?

Why would eye care providers offer me this plan at no charge?

Independent optometrists and ophthalmologists listed as providers recognize that not every family has insurance that provides affordable eye care, glasses and contact lenses. By extending to you and your family the EyeOne Platinum Vision Plan program, providers know that you will develop a relationship with their office and refer your friends to them.

 

How do I apply for my EyeOne Platinum Vision Plan membership?

You may enroll in the EyeOne Platinum Vision Plan by clicking Enroll on the home page and completing the request form.

 

Do I need to apply for each family member?

Please apply for a card for each adult family member that is 16 years of age or older. Children under 16 years of age may use the adult card.

 

How soon will I receive my EyeOne membership card?

Your card will be mailed to you within three business days, however you may request the EyeOne Platinum benefit at the time of visit if your card has not arrived, before your appointment.

 

How do I use my EyeOne membership card?

First, you will need to select an eye care provider from the list of Providers which can be accessed on the home page. Then you will need to call that office. The office staff will then work with you to schedule you an eye care appointment. Please let them know that you are an EyeOne Platinum Vision Plan member.

 

Are there other EyeOne Vision Plan Benefit Programs available?

EyeOne will soon launch an EyeOne Platinum Plus Vision Plan Benefits.
